Personalized Therapeutic Nutrition Programs

Send in your finished questionnaire to the address below, along with a check for $25 dollars, which is also good for the first payment toward your supplement if you choose to follow a plan.

Your nutritional evaluation, normally $80, is free if you then decide to follow your personalized supplement program. Make certain that you indicate on your form that you obtained it from this Website in order to get the $25 price. For those who choose not to use the supplements, you may consider submitting the questionnaire along with the $25 fee simply in order to assess whether your diet may be deficient in any of a number of the many essential nutrients, which ones they are, and what you need to do to start functioning at a much higher and healthier level.
